Case Details
This young woman from the Hurst region of Texas came to my Southlake office complaining of the profile appearance of her nose. She wanted to reduce her dorsal hump and narrow the width of the middle portion of her nose as seen on frontal view. She had been considering the changes she wanted for a significant length of time, and was happy with the appearance of her nasal tip. With the use of computer imaging, I had a discussion of her goals with herself and her parents to be sure we had the same expectations from surgery. Once I was certain of her desires, we decided to proceed with a closed rhinoplasty.
During the operation, I addressed the dorsum through the closed approach, which allowed me to externally watch the changes to the appearance to the nose with each step of the procedure. I carefully lowered the cartilaginous dorsum sharply, and then matched the bony height with the use of a rasp. This expectedly created an open roof, so I performed a low-to-low osteotomy via the intranasal route followed by infractures. Final refinements of the dorsum were performed and the operation was complete. Her recovery was uneventful, and she loves her new nose. Pictures were taken during her 6 month post operative check, and we can see the nice profile changes. Further refinement is expected as a small amount of dorsal edema abates, and the sidewalls narrow over the next six months or so.
