Eyelid Surgery
Many men and women are bothered by droopy or hooded eyelids, either due to aging, heredity, or both. The same is true of the wrinkles, puffiness and fat bulges that can develop in the lower eyelids. These conditions can be particularly troublesome since the eyes are such a critical part of our expressions and our interactions with others. Patients often complain of looking tired or unhappy despite feeling otherwise and inquire about blepharoplasty (eyelid surgery).
Eyelid surgery is frequently the most cost-effective means of correcting a “tired” look and improving overall facial appearance. Sometimes drooping eyebrows will exacerbate the sagging skin of the upper eyelid, necessitating a forehead lift for full rejuvenation of the eye area.

Surgical Techniques
One of the most common procedures performed by plastic surgeons in the DFW area is eyelid surgery, technically called blepharoplasty. This is because, unfortunately, the eye area is one of the first areas to show the signs of aging, such as sagging eyelid skin and puffy, wrinkled skin beneath the eyes. Since the eye area is so central to our facial expressions, surgery to rejuvenate the eyelids can have a tremendous impact on the overall appearance of the face.
Dr. Bogdan and Dr. Cason will take several factors into consideration when evaluating you for cosmetic eyelid surgery. First, they will need to determine if you truly have excessive eyelid skin, or if your sagging eyelids are caused mainly by a drooping brow. If this is the case, you may actually need an endoscopic brow lift, either with or without blepharoplasty. Also, many people who choose eyelid surgery also have rhinoplasty or a facelift.
Our surgeons will need to decide if you require upper eyelid surgery, lower eyelid surgery, or both, and what approach would be best suited to your condition. Then they will utilize the following surgical techniques:
Upper Eyelid Surgery
Upper eyelid surgery allows removal of excess skin and occasionally fat bulges that contribute to a fatigued, tired, or grumpy appearance. The incision generally follows the natural fold of the upper eyelid, extending just beyond the outside corner into the existing creases. Through this incision, a strip of excess skin is removed and fatty tissue is eliminated or repositioned to create a more refreshed, alert appearance. Because the incision follows the natural crease of the eyelid, it is usually very well-concealed and rarely noticeable.
Lower Eyelid Surgery
Lower eyelid surgery may be done through the inside of the lower eyelid, the so-called transconjunctival or “scar-less” technique. This technique requires no external incision, and is appropriate only for the removal or redistribution of fat. It cannot be used to remove excess skin, so is not the best choice if there is significant lower eyelid wrinkling.
In this case, a “classic” approach might be more appropriate, with an incision just underneath the eyelashes. Through this incision, excess skin, muscle and fat are removed, or fat may be redistributed to eliminate the puffy appearance. Other adjustments to correct specific problems such as muscle laxity may also be performed. The incision, as with other incisions used around the eyes, becomes essentially invisible as it heals. Another technique, which combines the internal incisions with fat grafting to blend the eyelid cheek junction is also available. Any extra skin and wrinkling can then be removed with a “skin-only” excision, or with skin resurfacing techniques to achieve a smooth lower lid contour.
